Because any type of dental 'Insurance' is a contract between you and the insurance company
and possibly your employer, we are only able to become involved to a limited degree.

It is ultimately the responsibility of each patient to determine if a given benefit plan permits them
to be treated with coverage at our office. This may require contact with the benefit carrier. The
correct question to ask is if your plan is a "free standing" plan that allows you to see the dentist
of your choice.

We do not accept Medicaid, Badger Care, Family Care, etc patients.

Dr. Cahlamer does accept the Federal Blue Cross Plan, Delta Premier and Delta Plus plans.

Dr. Morgenroth does accept the Federal Blue Cross plan and Delta Dental Premier (not the PPO)
plans. Again, it is your responsibility to verify that you can be treated and covered at this office.

Our office will be more than happy to complete any claim forms for services rendered and also to
submit pre-treatment estimates for major services needed. You just need to provide us with the
necessary information to do so such as carrier addresses, group numbers, contact numbers, etc.

We will propose the treatment necessary for your optimal dental health. Please do not let your
insurance coverage dictate your treatment as this can jeopardize your dental health.

Payment arrangements can be made for larger treatment cases. These will be handled by Kari
our office manager. Feel free to discuss insurance or financial matters with us any time.

We do also accept MasterCard and Visa.

We offer seniors (over age 65) a 5% discount on regular services or a 10% seniors cash discount
if payment for services is made at the time of treatment by cash or check (no credit cards).
